You can get the Czech Trade License cancellation form in two ways:
In-Person: Visit any Trade License Office (Živnostenský úřad) to pick up the paper form.
Online: Download the form online above, and complete it before submitting your application.
To complete the Czech trade license cancellation form, you need to provide the following:
Personal Details: Your full name, birth number, and ICO (business registration number).
Changes: Details about any changes to your business registration.
Pausing Information: If pausing, specify the start and end dates for the pause.
Cancellation Information: If canceling, enter the date of cancellation.
Residency Address: Updates to your business and residency address.
Professional License Info: Cancellation details related to the responsible representative and business unit.
Social Tax & Health Insurance: Cancellation details for social and health insurance.
Final Information: Date of application and your signature.
You have two options for managing your Czech trade license: cancel it or pause it.
Cancel: If you cancel your trade license, you will need to go through the full registration process to obtain a new one in the future.
Pause: If you pause your trade license, you can reactivate it later without having to re-register from scratch.
You can pause your trade license until December 31, 2099.
To cancel or pause a trade license, start at the Trade License Office (živnostenský úřad). Here, you must inform the office that you are pausing or canceling a Živno. Reporting changes is possible at any branch, and the office will provide a confirmation letter of any changes.
To cancel or pause your trade license, you must provide a valid passport, application form, ICO number, and support documents.
Go to the local Social Tax Office to cancel your registration to Czech social taxes.
Czech health insurance must be canceled at your medical insurance provider’s office: VZP, OZP, PVZP, etc.
The Zivno Trade License does not cancel automatically. This means that when you finish a project, lose work, or move back to your home country, you should cancel or pause the license. You should also notify and cancel with social security, health insurance, and social tax if applicable. Failing to do so will incur debt, as you are expected to make payments even with zero income or when not living in the country.
