What you need to provide
Passport
Two (2) passport photos*
Birth certificate or marriage certificate**
Document confirming the purpose of your stay - e.g. trade licence, study program enrolment document, marriage or birth certificate
Proof of your accommodation in the Czech Republic for at least 2 years (e.g. rental agreement, ownership deed, confirmation of accommodation from the property owner)
A signed power of attorney for our arrangement of your Czech long-term visa extension (to be completed at our office)
Comprehensive Health Insurance***
*Photos should be in colour and 3.5 cm x 4.5 cm with a white background.
**Needed only for the first extension
***If you don’t have Czech health insurance, we’re happy to arrange it for you.
