The Czech Blue Card application form is available in both Czech and English. However, since Czech is the official language, you should fill out the application form using Czech words.
If you're applying for a Czech Blue Card for the first time, you must apply at your local Czech embassy. If you already have a Blue Card from another EU country or are renewing your Czech Blue Card, submit your application at the Ministry of Interior (MOI) office in your residency district.
You can obtain the Czech Employee Card Application Form in following ways:
Paper Form: Pick it up in person at the Immigration Office at information window.
Download Online: Download the green application form above and complete it before your appointment.
To obtain a Czech Employee Card, you will need to provide:
Completed Application Form: Download it above.
Immigration Office Fee: Paid with value stamps (Kolky).
Accommodation Document: Rental contract or proof of accommodation.
Vacancy Number: Job vacancy number from the labor office.
Two ID Photos: 3.5 cm x 4.4 cm with a white background.
Purpose of Stay Documents: Work contract or amendment to the work contract.
Your Passport: Valid passport.
