The Czech Blue Card application form is available in both Czech and English. However, since Czech is the official language, you should fill out the application form using Czech words.

In-Person: Visit the Immigration Office to pick up the paper application form.

If you're applying for a Czech Blue Card for the first time, you must apply at your local Czech embassy. If you already have a Blue Card from another EU country or are renewing your Czech Blue Card, submit your application at the Ministry of Interior (MOI) office in your residency district.
For the Czech Blue Card application, you need to submit the following:
Completed Application Form: Download it above.
Two ID Photos: 3.5 cm x 4.5 cm with a white background.
Immigration Office Fee: 2,500 CZK, paid with value stamps (Kolky in Czech).
Accommodation Document: Rental contract or proof of accommodation.
Purpose of Stay Documents: Work contract.
Vacancy Number: Required if you don’t have free access to the labor market.
Education Documents: University diploma, translated into Czech if in a foreign language, and an apostille if applicable.
The Czech Blue Card is issued for a period of 3 years.
To complete the Czech Blue Card application form, you need to provide:
Vacancy Number: The job position’s vacancy number, labor office details, and validation.
Personal Information: Your details, marital status, contact information, education, and occupation.
Employment Information: Your current employer’s name, address, and employment period.
Previous Employment: Details of jobs with non-Czech employers before moving to Czechia, including company names and addresses.
Czech Residence: Your current address in the Czech Republic.
Previous Residency: Your permanent address outside Czechia.
Previous Stay in Czech Republic: Any previous stays in the country lasting more than 3 months, if applicable.
Arrival in the Czech Republic: Your arrival date and travel document details.
Family Members: Details and addresses of your spouse or children.
Parents' Details: Details and addresses of your parents, including your mother’s maiden name.
Siblings: Details and addresses of your siblings, if applicable.
Including your parents’ details, especially your mother’s maiden name, is important for generating your Czech birth number, which you will need for your employee tax return later. Make sure all information is correct to prevent problems with your Blue Card application.
Foreigners without free access to the labour market can only take job posts with vacancy numbers older than 30 days. This requires the employer to publicly announce the job, which provides time for Czech / EU citizens to respond first. If there is no interest in the vacancy after 30 days, the employer can then hire a foreigner under the older vacancy number.
